Your review was anonymous. I have reviewed my charts and have no record of seeing a patient with the type of situation to which you are referring. So I am proposing the following.

This will then allow me to review your actual chart and see if there was anything I could’ve done better to address your very serious allegations. This will benefit all future patients.
